Doctor Video Consultation Guide

Complete instructions for conducting, documenting, and completing tele-nephrology consultations.

Before

  • Log in to your doctor account using your registered email and password.
  • Find today's consultation — consultations assigned to you (where doctor_id matches your user ID) appear in the appointment system.
  • Open the consultation room at the consultation link. This is the Kidney Command Center — a three-panel workspace.

Review Patient Records (Left Panel)

Overview

Demographics, kidney summary, clinical alerts (high potassium, low eGFR, high BP, allergies)

Reports

Uploaded medical reports — click to open

Labs

Up to 8 recent lab result sets with key values

Meds

Active medications across all prescriptions

Passport

Kidney Passport — diagnosis, CKD stage, blood group, allergies, dialysis/transplant status

Timeline

Chronological list of labs and reports

  • Confirm the correct patient and appointment — the VideoHeader shows consultation type, status, doctor name, and patient name.
  • Check camera, microphone, speakers, browser permissions, and internet connection.
  • Use a private, quiet, well-lit location with a stable internet connection.

Starting

  • Open the assigned consultation room.
  • The center panel shows the VideoConsentScreen with four required acknowledgments.
  • Check all four boxes: Identity, Telemedicine Consent, Emergency Disclaimer, Privacy.
  • Click "Continue to Video" — the button enables only when all four are checked.

What happens when you join

  • The system calls the videoConsultation backend function with action: "join".
  • A Cloudflare RealtimeKit meeting is created (if one doesn't exist).
  • A participant token is generated using the doctor preset (your role is detected automatically).
  • The consultation status changes to "In Progress" and video_status to "active".
  • Allow browser camera and microphone access when prompted.
  • Wait for the secure video room to initialize ("Preparing secure video session…").
  • Confirm you joined as the doctor — the header shows "Doctor View".
  • If the patient has not joined yet, wait. Use the time to review records or begin documenting notes.

During

  • Confirm the patient's identity (full name, date of birth).
  • Confirm the patient's current physical location (for emergency response).
  • Confirm telemedicine consent verbally if your jurisdiction requires it.
  • Review uploaded reports and lab results in the left panel.

Clinical Workspace Tabs (Right Panel)

Notes

Structured SOAP notes (Reason, History, Examination, Assessment, Plan). Auto-saves 3 seconds after you stop typing. Assessment is required for completion.

AI Assistant

Seven actions: Summarize Reports, Extract Lab Values, Consultation Draft, Note Draft, Follow-Up Checklist, Flag Missing Info, Patient Explanation. Each draft has Edit, Approve, Discard buttons.

Prescription (Rx)

Diagnosis, advice, follow-up date, and medications (name, dose, frequency, duration, instructions).

Care Plan

Diet, fluid, BP target, diabetes, dialysis, transplant, warning signs, follow-up plan.

Follow-up

Date, recommended labs, imaging, referral, urgency (Routine/Soon/Urgent), patient instructions.

AI output is only a draft. Every AI-generated result carries an amber warning: "AI-Generated — Doctor review required. This is not a diagnosis." You must review, edit if needed, and approve before it enters the clinical record.

  • Mute/unmute and camera on/off are controlled within the RealtimeKit video interface.
  • Screen sharing depends on the doctor preset configuration in Cloudflare.
  • If connection drops, RealtimeKit auto-reconnects. Notes auto-save every 3 seconds — no data is lost.
  • You can document notes, prescriptions, and care plans while the video is active — the layout is designed for simultaneous use.

Ending

  • Use the doctor-only End button — available as a red "End" button in the header and a red "End Consultation" button in the Quick Action Bar.
  • Patients do not have an End button — only the assigned doctor or super admin can end the session.
  • A confirmation dialog appears: "End this consultation for all participants?"
  • Click "End Consultation" to confirm, or "Continue Consultation" to cancel.

What happens when you end

  • All participants are disconnected (Cloudflare RealtimeKit kick-all).
  • video_status is set to "ended" and video_ended_at is recorded.
  • The consultation status changes to "Clinical Documentation Pending".
  • A completion modal appears with a documentation checklist.

Ending the video does NOT automatically complete clinical documentation. The "Complete Consultation" button is disabled until the Assessment field in Notes is filled.

After

  • Complete and save notes — the Assessment field is required. Notes auto-save or click "Save".
  • Finalize prescription if needed — click "Save Prescription" or "Update Prescription".
  • Finalize care plan if needed — click "Save Care Plan" or "Update Care Plan".
  • Set follow-up date and requested labs — click "Save Follow-up".
  • Review all documentation using the tabs.
  • Click "Complete Consultation" only after the Assessment note is saved. This sets status to "Completed" and records completed_at.

Visible to Patient

  • • Prescription (diagnosis, medications, advice, follow-up date) — printable
  • • Care Plan (diet, fluid, BP, diabetes, dialysis, transplant, warning signs)
  • • Follow-up date on the prescription
  • • Consultation status "Completed" on Appointments page

Internal to Doctor

  • • Detailed SOAP clinical notes
  • • AI assistant logs and approval status
  • • Patient Intelligence panel data (internal alerts)

Checklist

  • Open the consultation at /consult/:consultationId
  • Review patient records (Overview, Reports, Labs, Meds, Passport, Timeline)
  • Check camera and microphone are working
  • Complete four consent acknowledgments
  • Click "Continue to Video" — join as doctor (automatic)
  • Allow browser camera and microphone access
  • Wait for video room to initialize
  • Confirm patient identity and current physical location
  • Confirm telemedicine consent verbally if required
  • Conduct consultation — use Notes, AI, Rx, Care, Follow-up tabs
  • Save notes (auto-saves after 3 seconds, or click "Save")
  • Finalize prescription — click "Save Prescription"
  • Finalize care plan — click "Save Care Plan"
  • Set follow-up date, labs, imaging, urgency — click "Save Follow-up"
  • Click "End" (header) or "End Consultation" (Quick Action Bar)
  • Confirm in the warning dialog
  • Ensure Assessment note is filled (Required)
  • Click "Complete Consultation"
